By participating in this Pilates class, I acknowledge that Pilates and other forms of physical exercise involve physical activity and may carry inherent risks, including, but not limited to, muscle soreness, strains, sprains, falls, or other injury.

I confirm that I am voluntarily participating in this class and believe that I am physically able to participate. I understand that I am responsible for informing the instructor before class of any injuries, medical conditions, physical limitations, or other circumstances that may affect my ability to safely participate.

I agree to listen to and follow the instructor’s directions and to modify or stop an exercise if I experience pain, discomfort, dizziness, or otherwise feel that I cannot safely continue. I understand that the instructor is not providing medical advice, diagnosis, or treatment.

Release of Liability

To the fullest extent permitted by applicable law, I release and agree to hold harmless the Pilates instructor, event organizers, venue, and their respective owners, employees, agents, and representatives from claims arising from my voluntary participation in the class, except to the extent caused by their gross negligence, willful misconduct, or any other liability that cannot legally be waived.

I understand that participation is voluntary and that I may choose not to perform any exercise or may stop participating at any time.

Emergency Medical Authorization

In the event of an emergency, I authorize the instructor or event organizers to seek appropriate emergency medical assistance on my behalf if I am unable to do so myself. I understand that I am responsible for any medical expenses resulting from an emergency.
